Health & Wellness Coordination Packages

Service Description

Engage in one-on-one sessions with our skilled Concierge Nurses to receive personalized education on various health topics to help prepare you to make informed decisions about your health and well-being. We offer two Concierge Health & Wellness Coordination Packages that include the following: • Weekly or Biweekly Concierge Nurse Home Visits • Weekly or BiWeekly Medication setup and management • Doctor's appointment patient advocacy visits (Our nurses attend the appointments with you.) • Vital signs monitoring each visit • Comprehensive nursing assessments • Concierge Healthcare coordination services (We will help manage all your healthcare needs.) • Assistance getting referrals for other needed services (such as PT, OT, specialists, etc..) • Phone access to Concierge Nurse for nursing related questions Monday-Friday from 8am to 6pm. *Health & Wellness coordination packages are a month-to-month contract.
